Debut Theatre Company (“Debut”), a Colorado nonprofit organization with primary offices located at 827 Riverside Avenue Fort Collins, CO 80524, relies on volunteers to assist with activities that may include—but are not limited to—set construction and deconstruction; moving, assembling and/or deconstructing furniture or equipment; organizing materials; cleaning, and other facility-related tasks (“Volunteer Activities”). These activities may take place onsite at our principal offices or offsite at any number of rented or donated spaces including, though not limited to, Poudre School District theaters, the Lincoln Center in Fort Collins, nonprofit or for-profit theaters, storage facilities or Debut’s AMC theater location at 2525 Worthington Circle, Fort Collins, CO 80526.

In consideration for being permitted to participate as a volunteer, I agree to the following terms:

1. Voluntary Participation

I acknowledge that my involvement with Debut is completely voluntary. I am not an employee of Debut and will receive no compensation, benefits, or insurance coverage for my volunteer work.

2. Assumption of Risk

I understand that Volunteer Activities may involve physical labor, use of hand or power tools, climbing ladders, lifting heavy objects, exposure to dust or paint, and other risks of injury. I knowingly and freely assume all risks, both known and unknown, associated with my volunteer work.

3. Release of Liability

To the fullest extent permitted by law, I release and hold harmless Debut, its directors, officers, employees, contractors, volunteers, and agents from any and all claims, liabilities, damages, or losses arising out of or related to my participation in Volunteer Activities, including claims based on the negligence of Debut.

4. Medical Treatment

I consent to receive medical treatment in the event of an injury or emergency and agree to be responsible for any medical costs incurred.

5. Insurance

I understand that Debut does not provide health, liability, or workers compensation insurance for volunteers. I am responsible for my own insurance coverage.

6. Safety & Conduct

I agree to follow all safety instructions from Debut staff or designated project leads, including proper use of tools, protective equipment, and materials.

7. Photographs & Media

I grant Debut permission to use photographs or videos of me taken during Volunteer Activities for promotional or educational purposes, without compensation. I hereby grant Debut a non-exclusive right and license to display, exhibit, transmit, broadcast, reproduce, record, photograph, digitize, adapt, create derivative works, and otherwise use my name, image, and likeness, and all materials created by or on behalf of Debut that incorporate any of the foregoing on a perpetual basis throughout the world, and in any medium or format whatsoever, whether now existing or hereafter created, without further consent from or royalty, payment, or other compensation to me for public relations purposes.

8. Minors

If the volunteer is under 18, a parent or legal guardian must sign below. Minors may participate only in age-appropriate tasks approved by Debut staff.

9. Termination of Services

I understand that Debut, without notice or hearing, may terminate my services as a Volunteer at any time, with or without reason.

10. Governing Law

This agreement shall be governed by and interpreted in accordance with the laws of the State of Colorado, without regard to its conflict-of-law principles. Any claims or disputes arising out of or related to this agreement or my volunteer activities shall be brought exclusively in a court of competent jurisdiction within the State of Colorado.

I understand that my electronic signature has the same legal effect as a handwritten signature under the Colorado Uniform Electronic Transactions Act (UETA) and the federal ESIGN Act.
I am signing this document voluntarily and knowingly.
If signing on behalf of a minor, I confirm that I am the child’s parent or legal guardian and have the authority to do so.
